29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) CSI Industries, Inc. - On or about, 4/12/23, and at times prior thereto, employees were operating the roller machine without guarding to prevent employees from coming in contact with the rollers. A recordable injury occurred on 4/12/23 while the employee was working at the rolls. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1904.41(a)(2): An establishment that had more than 20 and fewer than 250 employees at any time during the previous calendar year and the establishment is classified in an industry located in Appendix A, did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee, no later than the date listed in paragraph (c) of this section: a.) CSI Industries, Inc.; 6910 W. Ridge Road, Erie, PA: On or about 4/25/23, the employer did not submit the calendar year 2022 information from their OSHA Form 300A or equivalent by 03/02/2023. The establishment employed 45 employees and was classified 332322 during calendar year 2022. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). The emergency stop pedal had faded and was not painted red to indicate it was an emergency stop mechanism.
