29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): a) Jobsite, 5988 Irishtown Road, Bethel Park, PA 15102: Observed on or about April 26, 2023, an employee working at the bottom of an excavation that was approximately 6 feet 4 inches in depth with vertical sidewalls was not protected from cave-ins. The employee was exposed to an engulfment hazard while engaged in work at the bottom of the excavation.

There was no stairway, ladder, ramp or other safe means of egress for the employee working at the bottom of the excavation.
