29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): On or about May 9, 2023, the employer did not provide fall protection such as, but not limited to portable guard rail systems, while employees were working around an approximately 36 inch by 24 inch pit opening.
