29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids, or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  JOB SITE:924 Jenny Lee Dr. Pevely, MO: on or about 4/27/2023, the employer did not ensure employees used appropriate eye or face protection while using pneumatic nail guns while installing roof shingles. The workers were exposed to a struck-by hazard.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   JOB SITE: 924 Jenny Lee Dr., Pevely, MO: on or about 4/28/2023, the employer did not protect employees who have engaged in residential roofing operations 6ft (1.8m) or more above lower levels. The employees were conducting residential roofing operations on a single-family mobile home and were exposed to the hazard of falling approximately thirteen (13) feet.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  JOB SITE: 924 Jenny Lee Dr., Pevely, MO: On or about 4/27/2023, the employer did not ensure portable ladders were constructed and placed so three (3) feet or more were extended above the upper landing.  Workers performing residential construction were exposed to falling approximately thirteen (13) feet while using a straight ladder to ascend/descend from the ground level to the roof.
