29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:  a) Jobsite at 701 E 12th Street, Chester, PA - On or about April 25, 2023, the employer did not ensure that each working level was fully planked or decked between the front uprights and guardrail supports. An employee engaged in masonry activities was working from a scaffold platform that was not fully planked, thereby exposing employees to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  a) Jobsite at 701 E 12th Street, Chester, PA - On or about April 25 2023, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the second level of the scaffold platform, thereby exposing employees to fall hazards of approximately 14.5 feet.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) Jobsite at 701 E 12st, Chester, PA 19013 - On or about April 25, 2023, the employer did not ensure an employee working from the 2nd platform of a supported tubular welded frame scaffold, approximately 14.5 feet above the ground level, was protected from fall hazards by the use of a fall protection system.  The NCG Construction Group was previously cited for a violation of this occupational safety and health standard or its equivalent standard29 CFR 1926.451(g)(1)(vii), which was contained in OSHA inspection number1610306, citation number 1, item number 2 and was affirmed as a final order on 1-20-2023, with respect to a workplace located at 701 E 12st, Chester, PA 19013.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:  a) Jobsite at 701 E 12st, Chester, PA 19013 - On or about April 25, 2023, the employer did not train employees working from the second level of a supported tubular welded frame scaffold how to recognize the hazards associated with working from scaffolds and to understand the procedures to control or minimize those hazards, exposing employees to fall hazards of approximately 14.5 feet.  The NCG Construction Group was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.454(a), which was contained in OSHA inspection number1610306, citation number1, item number1 and was affirmed as a final order on 1-20-2023, with respect to a workplace located at 701 E 12st, Chester, PA 19013.
