29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Prohos, Inc. - Whitehouse, Ohio: On or about March 13, 2023, employees who operated the Amada horizontal band saw (model HA-400; s/n 40600662) were exposed to amputation injuries in that the unused portion of the saw blade was not guarded. b) Prohos, Inc. - Whitehouse, Ohio: On or about April 25, 2023, employees who operated the W.F. Wells & Sons, Inc. horizontal band saw (model W-9) were exposed to amputation injuries in that the unused portion of the saw blade was not guarded. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) Prohos, Inc. - Whitehouse, Ohio: On or about April 20, 2023, the employer failed to report an amputation injury to an employee. On March 13, 2023, an employee sustained a partial finger amputation injury; the employer did not report the amputation within twenty-four hours. No additional abatement information is needed for this item.
