On or about 4/28/2023, the employer did not ensure the walking/working surfaces on which its employees were to work had the strength and structural integrity to support employees safely. The employees were working on the roof of a single-family home at a height of approximately twenty-one (21) feet. The slipboard the employee was working on dislodged causing the employee and board to fall off of the roof.
