29 CFR  1910.132(d)(1)(i):When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazards identified.  CSW, Inc. - Sylvania, Ohio: On or about May 2, 2023, the employer did not perform an adequate hazard assessment to determine the proper protective equipment, and ensure its use, to be worn by employees who are exposed to eye injury and skin and respiratory irritation when working with chemicals for proof making operations.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.133(a)(1):Protective eye and face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:  CSW, Inc. - Sylvania, Ohio: On or about May 2, 2023, the employer did not ensure that employees who used chemicals such as, but not limited to, butanone, methyl isobutyl ketone, cyclohexanone, diisopropylbenzene, polyurethane methacrylate oligomer, methacrylic esters, anionic surfactant, isopropanol, and heptanes, wore the proper eye protection to protect against chemical splashes to the eyes.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR  1910.138(a):The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.  CSW, Inc. -  Sylvania, Ohio: On or about May 2, 2023, the employer did not ensure that employees who used chemicals such as, but not limited to, butanone, methyl isobutyl ketone, cyclohexanone, diisopropylbenzene, polyurethane methacrylate oligomer, methacrylic esters, anionic surfactant, isopropanol, and heptanes, wore the proper personal protective equipment for the hands.  Employees that handed and used the chemicals in the liquid polymer and mounting areas were exposed to skin irritations. 29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  CSW, Inc. - Sylvania, Ohio: On or about April 25, 2023, the employer did not develop and implement an energy control program consisting of training, procedures, and audits, to protect employees from hazardous mechanical energy sources.  Employees that performed setup tasks on equipment such as, but not limited to, the LE Sauer Machine Co. "Sampler" roller (s/n RDM 901-13) were exposed to amputation injury during the servicing task.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
