29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  Employees were exposed to fall hazards at 13337 W 180th St, Olathe, KS 66062.  Employees engaged in residential roofing activities were exposed to fall hazards of  21 feet from the roof's  eave  to the ground below.  Fall protection was not utilized.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee exposed to fall hazards to enable them to recognize the hazards of falling and the procedures to follow in order to minimize them.   Employees were exposed to fall hazards at the jobsite located at  13337 W 180th St, Olathe, KS 66062.  Employees engaged in roofing activities were exposed to fall hazards of approximately 21 feet from the roof's eave to the ground below.  The employees were not provided training to recognize fall   hazards and the  methods to minimize the hazards associated with falling from heights.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Employees were exposed to fall hazards at 13337 W 180th St, Olathe, KS 66062.  Employees engaged in residential roofing activities were exposed to fall hazards of approximately  9 feet  from the roof's eave  to the ground below while accessing and exiting the roof.  The portable ladder did not extend at least 3 feet above the upper landing surface and was not secured to the residence's structure.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:   Employees were exposed to fall hazards at 13337 W 180th St, Olathe, KS 66062.   Employees were exposed to fall hazards of approximately 9 feet from the roof's eave to the ground below.  Employees were not trained in the correct way to setup ladders.
