The employer failed to provide an exit sign illuminated with at least five foot-candles (54 lux) by a reliable light source or distinctive in color.

29 CFR  1910.305(g)(1)(iv)(B): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used to run through holes in walls, ceilings, or floors.  (a) 1195 Rock Creek Cir, Lafayette CO: On May 3, 2023,  and at times prior, employees are exposed to shock hazards while using equipment attached to flexible cord that was observed running through holes in the wall from the dispatch room to the fiber room.
