Location: 42 Pine Vale Road, Waltham, MA; On or about 05/05/2023, workers were observed engaged in roofing activities who were not correctly tied off to fall arrest systems or otherwise protected from falls by alternative fall protection systems. DM Construction Inc. DBA DM Construction Services Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1622931, citation number 1, item number 1 and was affirmed as a final order on 01/25/2023, with respect to a workplace located at 36 Clarke Street, Lexington, MA.
