29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: (a) Alsco, 3850 Elm St, Denver CO, Maintenance Room: On or about May 5, 2023, and at times prior, employees were exposed to electrical shock and fire hazards while working around exposed wires coming from a live junction box. The junction box had exposed wires from the missing cover.
