On or about May 5, 2023, and at times prior thereto and thereafter, the employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization that occurred on date May 4, 2023. The employer did not report to OSHA until On May 8, 2023. Crossmark, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1379193, citation number 001, item number 001, issued on February 21, 2019, and was affirmed as a final order on March 19, 2019, with respect to a workplace located at 10901 Lakeline Mall Drive, Austin, TX 78717.
