29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  On or about May 9, 2023, employees were engaged in residential roofing activities without the use of fall protection in Mapleton, IL.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about May 9, 2023, two employees were exposed to eye injuries while using pneumatic nail guns to install shingles on a residential roof and the employer did not ensure that eye protection was utilized.  D.T. Roofing and Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard: 29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 1559566, citation number 2, item number 1 and was affirmed as a final order on 12/13/2021, with respect to a workplace located at 309 W Copperfield, Dunlap, IL.  D.T. Roofing and Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard: 29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 1502941, citation number 1, item number 4 and was affirmed as a final order on 03/22/2021, with respect to a workplace located at 1420 N Main Street, Washington, IL 61571.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about May 9, 2023, employees were exposed to fall hazards greater than 6 feet, while accessing and exiting a residential roof via extension ladders that only extended 2.5 feet above the upper landing surface in Mapleton, IL.  D.T. Roofing and Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard: 29 CFR 1926.1053(b)(1) which was contained in OSHA inspection number 1502941, citation number 1, item number 6(a) and was affirmed as a final order on 03/22/2021, with respect to a workplace located at 1420 N Main Street, Washington, Illinois, 61571.
