29 CFR  1910.303(g)(1)(vii)(B): The employer did not ensure that outdoor electrical equipment was protected from accidental contact by vehicular traffic:  (a) National Park Service, Lake Mead Recreation Area, Maintenance, 101 Katzenbach Drive Boulder City, Nevada: On or about May 9, 2023, the employer exposed employees to potential 480 volt electrical hazards from two unprotected transforms while operating vehicles and other mechanized equipment in the yard area.

29 CFR  1910.305(a)(2)(ix): Lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lamp holder with a guard:  (a)	National Park Service, Lake Mead Recreation Area, Maintenance, 101 Katzenbach Drive Boulder City, Nevada: On or about May 9, 2023, the employer exposed employees to potential electrical hazards in the dive shop from unprotected fluorescent light bulbs in that the bulbs were installed directly over a storage rack with items placed approximately 8 to 12 inches below the bulbs.   (b)	National Park Service, Lake Mead Recreation Area, Maintenance, 101 Katzenbach Drive Boulder City, Nevada: On or about May 9, 2023, the employer exposed employees to potential electrical hazards in the roads and grounds shop from unprotected fluorescent light bulbs in that the bulbs were installed directly over a storage shelf with items placed approximately 4 to 16 inches below the bulbs.

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) National Park Service, Lake Mead Recreation Area, Maintenance, 101 Katzenbach Drive Boulder City, Nevada: On or about May 9, 2023, the employer did not ensure the relocatable power tap/power strip was installed and/or used in a workman-like manner and in accordance with the manufacture's recommendations. A power strip used to supply 110-120 volts to a microwave oven in the electronic shop was potentially overloaded, and exposed employees who operated the appliance and those working in the immediate vicinity, to electrical and/or fire hazards.
