Employees engaged in painting operations in shop 51 were exposed to hazards associated with, but not limited to, crystalline silica, zinc oxide, and isopropyl alcohol in that employees required to wear full face elastomeric respirators where not provided with a fit test annually.

29 CFR 1910.1053(f)(2)(i):The employer did not establish and implement a written exposure control plan: Carpe Diem Industries, LLC doing business as Colonial Surfaces Solutions located at 4599 Campbell Road in Columbus Grove, Ohio: On or about May 10, 2023 in that the employer did not implement an written exposure control plan that includes: (A) A description of the tasks in the workplace that involve exposure to respirable crystalline silica; (B) A description of the engineering controls, work practices, and respiratory protection used to limit employee exposure to respirable crystalline silica for each task; (C) A description of the housekeeping measures used to limit employee exposure to respirable crystalline silica. Employees engaged in painting operations in shop 51 were exposed to inhalation hazards associated with crystalline silica such as, but not limited to, cough, fatigue, shortness of breath, and chest pain.
