29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Carpe Diem Industries on or about May 8, 2023, at the address of 4599 Campbell Road, Columbus Grove OH: the employer did not ensure additions to the Toyota LP forklift such as an employer fabricated working platform used on the Toyota LP Forklift was evaluated and plated with manufacturers approval prior to use. Employees working from this platform to perform quality checks without the benefit of the manufacturer's evaluation, were exposed to associated with powered industrial vehicles, such as but not limited to falls from heights and overturning.
