29 CFR  1910.133(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, metals , liquid chemicals,  or potentially injurious light radiation:  In the shop area, employees were not wearing safety glasses /or appropriate eye protection and were exposed to impact hazards (flying metal particles) in the following areas:  1) May 11, 2023, behind the Ocean Avenger (Model # REJUVENATOR-CE / Serial # 3198) an employee was observed wearing prescription eyewear without side shields and not  American National Standards Institute (ANSI-87) rated safety eyewear.   2) May 11, 2023, behind the Ocean Avenger CNC Beam Drill Line, three employees were observed not wearing any type of safety glasses.

29 CFR  1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  On or about May 11, 2023, on the fabrication shop at the Ocean Avenger 60' Beam Drill Line's Operating Computer Console; the strain relief was broken and electrical wires were exposed. This condition exposed employees to electric shock hazards.
