29 CFR 1910.144(a)(1)(iii): Stop. Emergency stop bars on hazardous machines such as rubber mills, wire blocks, flat work ironers, etc., shall be red. Stop buttons or electrical switches which letters or other markings appear, used for emergency stopping of machinery shall be red. On or about May 14, 2023, at an establishment located at 3243 Whiting Road, In Stevens Point, Wisconsin; in the manufacturing department at paper machine 4, each of the coast stop buttons used as stop devices were not red as required. The buttons were white with rings around and red tape/material on the background.

29 CFR 1910.261(k)(1): Emergency stops. Paper machines shall be equipped with devices that will stop the machine quickly in an emergency. The devices shall consist of push buttons for electric motive power (or electrically operated engine stops), pull cords connected directly to the prime mover, control clutches, or other devices, interlocked with adequate braking action. The devices shall be tested periodically by making use of them when stopping the machine and shall be so located that any person working on the machine can quickly disconnect the machine from the source of power in case of emergency. On or about May 14, 2023, at an establishment located at 3243 Whiting Road, in Stevens Point, Wisconsin; in the Manufacturing Department at Paper Machine 4, the employer did not have an emergency stop device at each of the dryer sections exposing employees to caught in hazards. The coast stop button on the equipment was not tested periodically to ensure it would stop the machine in case of an emergency.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. (a) On or about May 14, 2023, at an establishment located at 3243 Whiting Road in Stevens Point, Wisconsin, in the Manufacturing Department at Paper Machine #4, at the Second Dryer #10 can, the yellow rope return had an unguarded in-running nip point around the sheaves wheel exposing employees to caught between hazards. (b) On or about May 14, 2023, at an establishment located at 3243 Whiting Road in Stevens Point, Wisconsin, in the Manufacturing Department at Paper Machine #4, at the Second Dryer #10 can, the blue rope return had an unguarded in-running nip point around the sheaves wheel exposing employees to caught between hazards.

29 CFR 1910.261(b)(1): Lockouts. Devices such as padlocks shall be provided for locking out the source of power at the main disconnect switch. Before any maintenance, inspection, cleaning, adjusting, or servicing of equipment (electrical, mechanical, or other) that requires entrance into or close contact with the machinery or equipment, the main power disconnect switch or valve, or both, controlling its source of power or flow of material, shall be locked out or blocked off with padlock, blank flange, or similar device. On or about May 14, 2023, at an establishment located at 3243 Whiting Road, in Stevens Point, Wisconsin; in the Manufacturing Department at Paper Machine 4, during the cold start up process, the tail broke and wrapped around the first dryer can. Lockout devices were not utilized prior to entering the first dryer can exposing employees to caught between hazards.

Either a permanent guardrail or apron guard or both shall be installed in front of the first dryer in each section in accordance with paragraph (b)(1) of this section. On or about May 14, 2023, at an establishment located at 3243 Whiting Road, in Stevens Point, Wisconsin; in the Manufacturing Department at Paper Machine 4, the employer did not install a guard to prevent access to the first dryer in each section exposing employees to caught between hazards.
