The employer fails to ensure that all electrical equipment is utilized according to the listing or labeling.  a) This most recently occurred on or about May 15, 2023, in the office of Dollar General Store No. 9959, located at 404 East Pioneer Street, Crandon, Wisconsin; employees were exposed to electrical fire and shock hazards when a relocatable power tap was connected in series into another relocatable power tap.  Dolgencorp, LLC DBA Dollar General was previously cited under the name DG Retail, LLC DBA Dollar General Store #17668 for a SERIOUS violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 1572839, citation number 1, item number 1, and was affirmed as final order on May 23, 2022, with respect to a workplace located at 231 North Main Street, Troy, NH 03465. The employer fails to ensure that flexible cords and cables are not used as a substitute for the fixed wiring of a structure.  a) This most recently occurred on or about April 19, 2023, in the Break Room of Dollar General Store No. 9959, located at 404 East Pioneer Street, Crandon, Wisconsin; an employee was exposed to electrical fire and shock hazards where a flexible cord plugged into a coffee maker was being used as a substitute for fixed wiring.
