29 CFR  1910.22(d)(2): Hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again.  If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired; and  (a) On or about May 16, 2023, at the automatic nickel plating dip tank line,  dip tank line operators used a Werner Aluma-plank scaffold deck support  that was damaged by being bent and flared out at one end to access areas above open dip tanks to make adjustment to the process or perform other line operation tasks. This allowed the walkway to shift and be unbalanced with employees on it, exposing them to hazards associated with falling into the dip tanks.

29 CFR  1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system.  (a)  On or about May 16, 2023, at the automatic chrome plating dip tank line,  dip tank line operators accessed walk way areas above dip tanks to make adjustment to the process or perform other line operation tasks without fall protection to protect employees from falling into heated liquids and corrosive liquid materials in dip tanks, exposing employees to burn hazards from corrosive liquids or heated dip tank process liquids.  (b) On or about May 16, 2023, at the rack loading station, where employees loaded part racks onto the parts processing bar for the chrome dip tank line were exposed to a fall hazard into a five foot deep dip tank holding pit or into final stage hot water rinse tank.    (c) On or about May 16, 2023, at the tin/silver plating dip tank Line, Dip tank line operators access walk ways next to dip tanks to make adjustment to the process or perform other line operation tasks without fall protection in place to protect employees from falling into dip tanks, exposing employees to burn hazards from corrosive liquids or heated dip tank process liquids.

29 CFR  1910.219(f)(3):Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  (a) On or about May 16, 2023, at the automatic tin/silver plating line,  sprockets attached to a motor for the agitator part bar were not guarded to prevent contact with the rotating sprocket assembly, exposing line operators to an ingoing nip point and amputation hazards.
