At 6111 15th St E, Bradenton, FL: On or about 05/16/2023, the employer exposed employees to amputation and crushed hazards, in that employee was operating a shear machine to cut sheets of aluminum, during which time the machines power transmission apparatus at the rear of the machine were not guarded.

At 6111 15th St E, Bradenton, FL: On or about 05/16/2023, the employer exposed employees to amputation hazard, in that employees were operating a ten-foot press brake machine to bend sheets of aluminum, during which time the machines press-brake die was not guarded. b. At 6111 15th St E, Bradenton, FL: On or about 05/16/2023, the employer exposed employees to amputation hazard, in that employees were operating an eight-foot press brake machine to bend sheets of aluminum, during which time the machines press-brake die was not guarded. c. At 6111 15th St E, Bradenton, FL: On or about 05/16/2023, the employer exposed employees to amputation hazard, in that employees were operating a six-foot press brake machine to bend sheets of aluminum, during which time the machines press-brake die was not guarded. d. At 6111 15th St E, Bradenton, FL: On or about 05/16/2023, the employer exposed employees to amputation hazard. in that employees were operating an ironworker machine to cut pieces of aluminum, during which time the machines cutting blade bit was not guarded. e. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms.
