On or about 5/8/23, workers were exposed to falls while installing roofing materials onto a 4:12 pitch roof with a ground to eave height of 13 feet and on a 6:12 pitch roof with a ground to eave height of 23 feet without fall protection. The employer did not ensure the exposed employees were protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure.
