29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:  a) Media Manufacturing Department - On or about 05/17/2023, and at times prior, the employer exposed employees to trip hazards in that, the employer failed to ensure that walking-working surfaces were maintained in a clean and sanitary condition while employees were mixing chemicals such as Potassium Permanganate, Activated  Carbon and Alumina and packing the medias.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  a) Media Manufacturing Department - On or about 05/17/2023, and at times prior, the employer exposed employees to respiratory hazards in that, the employer failed to ensure that a medical evaluation was provided to employees to determine their abilities to use respirators while required to utilize half mask respirators for processing the media line and working with hazardous materials such as, but not limited to, Potassium Permanganate, Activated  Carbon and Alumina.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator:  a) Media Manufacturing Department - On or about 05/17/2023, and at times prior, the employer exposed employees to respiratory hazards in that, the employer failed to ensure that fit testing was provided to employees prior to their initial use of the tight-fitting facepiece respirators when employees required to wear half mask respirators while working with hazardous materials such as, but not limited to, Potassium Permanganate, Activated  Carbon and Alumina.

29 CFR  1910.134(h)(1)(i): Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in a sanitary condition:  a) Media Manufacturing Department - On or about 05/17/2023, and at times prior, the employer exposed employees to respiratory hazards in that, the employer failed to ensure that respirators issued for the use of employees were maintained in a sanitary condition when employees were wearing half mask respirators while processing the media line and working with hazardous materials such as, but not limited to, Potassium Permanganate, Activated  Carbon and Alumina.
