29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee use appropriate eye / face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  14320 Potranco Road, San Antonio. TX: On or about May 17, 2023, and at times prior thereto, employees were exposed to flying particle hazards when operating pneumatic nail guns and other hand powered tools without protective eyewear while performing framing installation.

29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  14320 Potranco Road, San Antonio. TX: On or about May 17, 2023 and at times prior thereto, employees were exposed to fall hazards while riding inside a man-made wooden box platform attached to a rough terrain telehandler that was not designed for such use by the manufacturer.

29 CFR  1926.451(c)(2)(v):The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: Note:  Preamble to Subpart L promulgated new paragraph (c)(2)(iv) & (v) in final rule to provide guidance. Added provision requires that in the case of fork-lifts, the entire scaffold platform be secured to the forklift:    14320 Potranco Road, San Antonio. TX: On or about May 17, 2023 and at times prior thereto, employees were exposed to fall hazards while riding inside a man-made wooden box platform attached to a rough terrain telehandler that was not secured to the telehandler forks.
