29 CFR  1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  On or about December 12, 2025, on the south side of a commercial building, an employee utilized the rails of a scissor lift as a platform to gain access to the building trusses.

29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:  On or about December 12, 2025, on the north end of a commercial building's wall, an employee operated an aerial lift with no safety lanyard attached to the basket.

Russell Bankston Construction dba: RB Framing LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1):, which was contained in OSHA inspection number 1670997, citation number 1, item number 1 and was affirmed as a final order on (10-03-2023), with respect to a workplace located at 14320 Potranco Rd. San Antonio, TX 78253.
