29 CFR  1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  At a worksite located at 415 Metallic road, Sedalia, MO, employees were exposed to struck by and crushed by hazards.  The employer exposed the employees to these hazards by allowing an employee to operate a powered industrial truck without first ensuring that the operator was certified to operate the equipment.

The employer exposed the operators to these hazards by allowing them to work near an unguarded horizontal shaft while operating the mill.

29 CFR  1910.305(c)(4):Faceplates for flush-mounted snap switches. Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface:  In the weigh room of the facility located at 415 Metallic road, Sedalia, MO, employees were exposed to electrical shock hazards.  The employer exposed the employees to these hazards by allowing employees to use a light switch that did not have a faceplate installed that completely covered the opening and sat against the finished surface.

The employer exposed the employees to these hazards by allowing them to handle chemicals to include Calsol 810 and TALC, as well as others, without first developing and implementing a written hazard communication program for the handling of the chemicals.
