29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least: On or about May 18, 2023, and at time prior thereto, employees used compressed oxygen and fuel gas cylinders. The employer did not ensure that the Oxygen compressed gas cylinders were separated when not in use. This condition exposed employees to fire, burns and struck-by hazards.

29 CFR 1910.1200(j)(2)(ii): The employer did not update their written hazard communication program required by paragraph (h)(1) by June 1, 2016 to reflect changes in labeling and safety data sheets: On or about May 18, 2023, and at time prior thereto, employees were involved in working with and around hazardous chemicals and materials. The employer had not updated hazard communication program, to include the Safety Data Sheets per Globally Harmonized System (GHS) and provide additional employee training on those Safety Data Sheets of those hazardous materials.
