29 CFR 1926.501(a)(2): Employees were allowed to work on surfaces which did not have the requisite strength and structural integrity:  On May 10, 2023, employees of BAM Contracting LLC were installing truss sections onto a building located at Preservation Square Community Building, North 16th Street in Saint Louis, Missouri. The employer did not ensure the work surfaces from which employees were working from had the strength and structural integrity.  Employees walking and working inside inadequately braced parallel cord trusses approximately 12 feet above the lower level were injured when the trusses employees were standing on collapsed and fell.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above the lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On May 10, 2023, employees of BAM Contracting LLC were installing truss sections onto a building located at Preservation Square Community Building, North 16th Street in Saint Louis, Missouri. The employer did not ensure that employees utilized fall protection systems while working at heights of approximately 12 feet. The employees were working from the wall top plate and walking inside the trusses without fall protection systems. Two employees were injured when truss sections fell to the ground.   BAM Contracting LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1345365, citation number 2, item number 1, and was affirmed as a final order on 10/19/2023, with respect to a workplace located at Altair at the Preserve Luxury Apartments 100 Winghaven Pointe Drive in O'Fallon, Missouri.
