29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.   On or about April 21, 2023, at 111 South Main Street in Waupaca, Wisconsin, an employee was exposed to potential struck-by hazards from falling roofing debris while walking near a trash trailer below a roof eave without using a hard hat.  Roofing debris was manually thrown off the roof surface down to the trash trailer.

29 CFR  1926.501(c)(3):  Protection from falling objects.  When an employee is exposed to falling objects, the employer shall have each employee wear a hard hat and shall barricade the area to which objects could fall, prohibit employees from entering the barricaded area, and keep objects that may fall far enough away from the edge of a higher level so that those objects would not go over the edge if they were accidentally displaced.  On or about April 21, 2023, at 111 South Main Street in Waupaca, Wisconsin, employees were exposed to potential struck-by hazards from falling roof debris while walking in area of debris trailer without an area barricade installed.  Roofing debris were falling from eave with the trailer in area of the roof access/egress ladder and parked work vehicles.

29 CFR  1926.503(b)(1):  The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record.  The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.   On or about April 21, 2023, at 111 South Main Street in Waupaca, Wisconsin, the employer did not provide required fall protection safety training certification for employees who were exposed to fall hazards greater than six feet while working on a roof surface with unprotected edges.  The C&C Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(b)(1), which was contained in OSHA inspection number 1490876, citation number 02, item number 001, and was affirmed as a final order on October 23, 2020, with respect to a workplace located at W18105 Hemlock Road; Wittenberg. WI  54499.
