29 CFR 1910.179(e)(4): Bridge truck(s) were not equipped with sweep(s) which extended below the top of the rail and projected in the front of the truck wheel(s): (a) On or about May 24, 2023, sub-contracted employees were exposed to a struck-by hazard in that both sets of bridge trucks on the Whiting gantry crane were not equipped with sweeps which extended below the top of the rail and projected in the front of the truck wheels.

29 CFR 1910.179(f)(4)(ii): Brakes were not able to be applied by mechanical, electrical, pneumatic, hydraulic, or gravity means. (a) This violation occurred on May 24, 2023, where subcontracted employee(s) working on the deck were exposed to a crushed-by hazard in that the hydraulic-electric parking brake was inoperable and could no longer be applied to hold the crane whenever the operator left the cab.
