This most recently occurred on or about May 30, 2023, during the removal shingles at a residential home located at 21940 Norton Road, Broadview Heights, OH 44146: Employees are exposed to a fall of 12 feet above the ground were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about May 30, 2023, the employer failed to ensure that the extension ladder used by employees to gain access to a residential roof located at 21940 Norton Road, Broadview Heights, OH 44146, rails extended at least 3 feet above the upper landing surface. Aden Miller DBA AM Roofing & Construction LLC, AM Roofing & Remodeling, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1459925, citation number 1 item number 2 and was affirmed as a final order on 3-13-2020, with respect to a workplace located at 518 Wallings Road Broadview Heights, Ohio 44147.
