29 CFR  1926.416(a)(1): The employer permitted employees to work in close proximity to energized and unguarded  electric power circuits that the employee could contact in the course of their work:   On or about March 21, 2025, at the job site located at 1916 26th Street, Cuyahoga Falls, Ohio 44222; during the installation of roofing material on a residential two family dwelling home, the employer did not ensure employees who were using a portable aluminum ladder were at a safe distance from the energized power line (7620 volts) above head at their work location.

29 CFR 1926.501(b)(10):"Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.  On or about March 21, 2025, during the tear-off of roofing material at a job site of a residential two family dwelling located at 1916 26th Street, Cuyahoga Falls, Ohio 44222, the employer did not ensure employees working from a low-sloped roof were protected from a fall of approximately 10 feet by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about March 21, 2025, the employer failed to ensure that the extension ladder used by employees to gain access to the roof of the residential two family dwelling located at 1916 26th Street, Cuyahoga Falls, Ohio 44222, extended at least 3 feet above the upper landing surface.   Aden Miller DBA AM Roofing & Construction LLC, AM Roofing & Remodeling, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1673359, citation number 2 item number 1 and was affirmed as a final order on 7-24-2023.
