29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy.    On or about May 31, 2023, the employer did not ensure that the lockout tagout procedure for the "Haeger" press clearly and specifically outlined the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy for operators performing set-up tasks. The press was not shut down or locked out prior to employees engaging in set-up tasks, exposing them to pinch/nip point hazards and amputation injuries.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):  On or about May 31, 2023, the employer did not ensure ensure that employees implemented energy control application steps prior to allowing them to perform set-up tasks on the "Haeger" press, such as tooling changes.  The "Haeger" was not shut down or turned off prior to employees engaging in this servicing and maintenance activity, exposing them to pinch/nip point hazards and amputation injuries.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.   On or about May 31, 2023, the employer did not provide training to employees performing servicing and maintenance tasks on the Haeger press machine. Employees do not perform lockout tagout for these tasks, exposing them to pinch/nip point hazards and amputation injuries.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle.  On or about May 12, 2023, the employer did not ensure "The Haeger" press had guarding or a means to prevent operators from having any part of their body in the danger zone during the operating cycle.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees.   On or about May 31, 2023, the ER had 8 instances in the Forming Department in which electrical equipment was not free from recognized hazards exposing employees to electrical shock hazards. The following instances were observed:  (a)	On or about May 31, 2023, the foot pedal for "The Haeger" had unprotected cable entering through a metal box to supply power to the metal foot pedal. There was no fitting to connect the cable to the foot pedal. Instead, there was electrical tape around the cable at the connection point. (b)	On or about May 31, 2023, Auto-Sert #2 Foot Pedal had unprotected conductors entering through a metal box to supply power to the metal foot pedal. The exterior cable jacket insulation was stripped back exposing the conductors. The only protection was the wire individual wire insulation. There was no fitting to connect the cable to the foot pedal. (c)	On or about May 31, 2023, Auto-Sert #3 Foot Pedal Box had damage to the exterior cable jacket insulation just before the connection to the Foot Pedal Box. The only protection was the wire individual wire insulation. (d)	On or about May 31, 2023, the foot pedal for Auto-Sert #3 had unprotected cable entering through a metal box to supply power to the metal foot pedal. There was no fitting to connect the cable to the foot pedal. Instead, there was duct tape around the cable at the connection point. (e)	On or about May 31, 2023, the Auto-Sert #3 power supply to Auto-Sert #3 Foot Pedal from the press had the exterior cable jacket Insulation stripped back, exposing the wires within. The only protection was the wire individual wire insulation. (f)	On or about May 31, 2023, the foot pedal for Auto-Sert #5 had unprotected cable entering through a metal box to supply power to the metal foot pedal. There was no fitting to connect the cable to the foot pedal. The exterior cable jacket insulation was stripped back exposing the conductors. The only protection was the wire individual wire insulation. (g)	On or about May 31, 2023, the Auto-Sert #05 Power Supply to pressing tool/throat had damage to the exterior cable jacket insulation and there was masking tape wrapped around at the connection point to the pressing tool/throat.  (h)	On or about May 31, 2023, the cable connection to a suspended junction box in between The Haeger, Auto-Sert #2, and Auto-Sert #3 had damage to the exterior cable jacket insulation exposing the conductors within.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  On or about May 31, 2023, the employer did not ensure that extension cords were used in accordance with the instructions included in the listing or labeling, exposing employees to electrical shock hazards. The following instances were observed:   (a)	There was power strip sitting at Auto Sert Press #05 plugged into a residential extension cord with multiple appliances plugged into the power strip. The extension cord ran 10-15 feet around product to a receptacle.  	 (b)	There was a power strip tied around electrical conduit with multiple extension cords plugged into it, directly below electrical panel LPH. 	 Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
