29 CFR 1910.147(f)(1)(i): WHEN LOCKOUT OR TAGOUT DEVICES WERE TEMPORARILY REMOVED FROM THE ENERGY ISOLATING DEVICE AND THE MACHINE OR EQUIPMENT WAS ENERGIZED TO TEST OR POSITION THE MACHINE, EQUIPMENT OR COMPONENT THEREOF, THE SEQUENCE OF ACTIONS IN 29 CFR 1910.147(f)(1)(i) THROUGH (f)(1)(v) WERE NOT FOLLOWED: a) At the jobsite, an employee was conducting maintenance on the SS-RO-1 (TXLCI) conveyor belt and a lock was not applied to isolate all energy sources, exposing the employee to the hazard of being caught in the conveyor belt.
