29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed: On or about August 17, 2023, an employee was exposed to machine hazards associated with the in going nip point created between the LF1-1 conveyor belt and shaft and the employer failed to require the employee be removed from the work area or provide alternative employee protection when testing or positioning the conveyor belt.
