29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a)	Brown's Roofing & Construction. On or about June 1, 2023 at the address of 740 Park St. Sandusky OH 44870: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b):  a)	Brown's Roofing & Construction. On or about June 1, 2023 at the address of 740 Park St. Sandusky OH 44870: The employer did not ensure employees were protected from falls while performing roofing work activities (installing asphalt shingles) atop a two-story multi-family residential structure with an 8 -12 roof pitch. Employees operating in this conditions were exposed to fall injuries from heights between  13 - 25 feet to the ground below.  Brown's Roofing & Construction was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1520547, citation number 1, item number 1 and was affirmed as a final order on July 13, 2021 with respect to a workplace located at Lot Northeast of Alexandrias Dr. and Lauras Ln. Intersection  Sandusky OH, 44870.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)	Brown's Roofing & Construction. On or about June 1, 2023 at the address of 740 Park St. Sandusky OH 44870: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing activities from elevated surfaces greater than six feet from the level below.  Brown's Roofing & Construction was previously cited for a violation of this Occupational Safety and Health Standard 1926.503(a)(1), which was contained in OSHA inspection number 1520547, citation number 1, item number 2 and was affirmed as a final order on July 13, 2021 with respect to a workplace located at Lot Northeast of Alexandrias Dr. and Lauras Ln. Intersection  Sandusky OH, 44870.
