Horning Construction LTD, on June 1, 2023 conducting work at the address of 5581 SR-53 Tiffin, Ohio 43883: The employer did not ensure the scaffolding platform used to elevate employees via a telehandler forklift, was designed by and used in accordance of a qualified person. Employees performing wood framing from the platform in this condition were exposed to falls of approximately 17 feet to the ground below.

29 CFR  1926.451(c)(2)(v):The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied.  a). Horning Construction LTD, on June 1, 2023 conducting work at the address of 5581 SR-53 Tiffin, Ohio 43883: The employer did not ensure the scaffolding platform used to elevate employees via a telehandler forklift, was securely attached to the forks. Employees performing wood framing from the platform in this condition were exposed to falls of approximately 17 feet to the ground below.

Horning Construction LTD, on June 1, 2023 conducting work at the address of 5581 SR-53 Tiffin, Ohio 43883: The employer did not ensure employees working from scaffolding platform elevated via a telehandler forklift were protected from falling to a lower level. Employees performing wood framing from the platform in this condition were exposed to falls of approximately 17 feet to the ground below without the benefit of fall protection.
