29 CFR 1910.252(a)(3)(i): Welding, cutting, or other hot work was performed on used drums, barrels, tanks, or other containers that had not been cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substances such as greases, tars, acids, or other materials which where subjected to heat, might produce flammable or toxic vapors: On or about May 26, 2023, six contractor employees conducting welding/pipefitting activities on the CP-2 (-45�F) ammonia storage vessel were exposed to burn hazards in that the host employer failed to ensure with absolute certainty that all flammable materials/substances were removed from the used container prior to authorizing hot work on the vessel. This failure led to the flash incident that resulted in six employees sustaining severe burns to the face and body.
