29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  On or about August 29, 2023, and at times prior thereto; the employer did not maintain an audiometric testing program. This condition exposed employees to health hazards such as hearing loss.  a) An operator in Department 7 was exposed to daily noise exposure or an equivalent sound level of approximately 87.8 dBA during the 470-minute sampling period on 8/29/2023. Exposure calculations included a zero increment for the 10 minutes not sampled.   b) An operator in Department 7 was exposed to daily noise exposure or an equivalent sound level of approximately 89.8 dBA during the 470-minute sampling period on 8/29/2023. Exposure calculations included a zero increment for the 10 minutes not sampled.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  On or about June 06, 2023, the employer did not provide suitable facilities for quick drenching or flushing of the eyes and body near corrosive Power Wash-B and Whitetail Industrial Services DEGREEZ-6 storage and use. This condition exposed employees to highly injurious corrosive chemical burn hazards in the following instances:  a) In the Can Washing Area, Dept. #3, employees use Power Wash-B, a highly injurious corrosive chemical (pH of 13.4-13.9), exposing employees to severe skin burns and severe eye damage.                                                            b) In Department #7, employees use Whitetail Industrial Services DEGREEZ-6, a highly injurious corrosive chemical (pH 12.5 - 14), exposing employees to severe skin burns and severe eye damage.

29 CFR  1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with the product identifier and word, pictures and/or symbols which provide the general information regarding the hazards of the chemical:  On or about June 06, 2023, and at time prior thereto, the employer did not ensure the proper labeling of several containers that were used to store different types of oils and chemicals. This condition exposed employees to health hazards.  a) In Department 7 (Davenport area), there are four (4) unlabeled plastic bottles on top of the Davenport machines that contain Ecocut Machine Oil.   b) In Department 7 (Davenport area), there is one (1) unlabeled plastic bottle on top of the Davenport machines that contained a mixture of Mineral Spirits and Ecocut Machine Oil.   c) In Department 7 (Welding area), there are five (5) unlabeled plastic spray bottles hanging on a rack from the trigger, bottle 1 contains soap and water, bottle 2 contains Degreez 6, bottle 3 contains Spindle Oil, bottle 4 contains Zep Degreaser, and bottle 5 is empty.   d) In Department 1, there are three (3) unlabeled plastic bottles located under a table used for work that contain Xtreme Cut 251C cutting fluid.   e) In Department 1, there is one (1) unlabeled plastic bottles on top of surface grinder machine that contains Extreme Cut 251 cutting fluid.
