29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: (a)Qvest Sanitation, LLC at 803 North Derby Lane, North Sioux City, SD 57049: On or about and at times prior to June 2, 2023, the exposing employer did not ensure adequate guarding was in place on the Line 1 Transfer Conveyor. This condition exposed employees to caught-in and nip-point hazards associated with the unguarded sprockets on the south end of the conveyor.
