29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  JOB SITE: 4800 Delmar, St. Louis, MO, On or about 6/6/2023, the employer did not ensure employees worked on a scaffold that was fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  Employees were installing a brick fa�ade from a scaffold on the side of an apartment building over ten (10) feet high without the scaffold being fully planked.

29 CFR  1926.451(b)(6): On scaffolds where scaffold planks are abutted to create a long platform, each abutted end did not rest on a separate support surface:  JOB SITE: 4800 Delmar, St. Louis, MO, On or about 6/6/2023, the employer did not ensure each abutted end of the scaffold plank rested on a separate support surface. Employees were working on scaffold planks over ten (10) feet high that were being supported by other scaffold planks.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  JOB SITE: 4800 Delmar, St. Louis, MO, On or about 6/6/2023, the employer did not ensure employees used portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface. Employees were using the scaffold's crossbraces to access the upper and lower levels of the scaffold.

29 CFR  1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy, or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.  JOB SITE: 4800 Delmar, St. Louis, MO, On or about 6/6/2023, the employer did not ensure that each employee on a scaffold was protected from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. Employees were working from a seven (7) level scaffold to install a brick fa�ade on the side of a five (5) story apartment building without having any additional protection from falling objects.

JOB SITE: 4800 Delmar, St. Louis, MO, On or about 6/6/2023, the employer did not ensure employees were protected from falling off scaffolds over ten (10) feet high.  Employees, we're installing a brick fa�ade from a scaffold on the side of an apartment building over ten (10) feet high without any fall protection.   Larios Masonry LLC was previously cited for a violation of this occupational safety and health standard or it's equivalent standard 29 CFR  1926.451(g)(1), which was contained in OSHA inspection number 1511787, citation number 1, item number 1 and was affirmed as a final order on 1/21/2021, concerning a workplace located at 1346 Hawthorne St., Saint Louis, MO 63105.
