29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about August 31, 2023, the employer did not develop, document, or utilize written procedures for the control of potentially hazardous pneumatic, hydraulic, and thermal energy during servicing and maintenance of Bekum BM705D blow molding machines designated as B-0 through B-6 at the workplace. An employee performed servicing and maintenance work on machine B-1, S/N: 204705-5-68, while reinstalling the head of the Blow Molder and performed work on pneumatic and hydraulic lines of "B-1", including installing a pneumatic transfer pin without isolating the machine from plant air or depressurizing the pneumatic system, and tightening a hydraulic line after it leaked without shutting down and depressurizing the hydraulic system.

29 CFR 1910.303(g)(2)(i):Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) On or about August 8, 2023, the employer did not ensure that electrical connections powering heating bands on the head of the Uniloy U-2 above the top platform were guarded against accidental contact, exposing employees cleaning the blowpin tip to electrical hazards crated by these live parts.
