29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.) Advantage Sales & Supply Co., LLC; on or about 06/08/2023 and at times prior thereto,  employees were working at and around 75T ENR machine #1 which did not have machine guarding in place to prevent employees from contacting moving parts. The moving parts include, but were not limited to, yarn and yarn spool area.  b.) Advantage Sales & Supply Co., LLC; on or about 06/08/2023 and at times prior thereto,  the moveable shields installed on the 75T ENR machines following the September 2022 near miss were not adequate. The moveable plexiglass guards could be easily bypassed and did not fully extend the length of the machine to protect the operators.

Advantage Sales & Supply Co., LLC; on or about 06/08/2023 and at times prior thereto, employees were designated to perform servicing and maintenance on equipment including, but not limited to, the ENR machines.  The employees were not trained as an authorized employee.

Advantage Sales & Supply Co., LLC; on or about 06/08/2023 and at times prior thereto,  Two maintenance employees were designated to perform servicing and maintenance on equipment including, but not limited to, the ENR machines in the Sew Area. Only one maintenance employee applied a lockout tagout lock and tag.
