Advantage Sales & Supply Co., LLC 99 Servidea Drive Ridgway, PA 15853on or about December 4, 2024, and times prior thereto: The employer did not ensure that all rack columns were anchored to the floor in accordance with the manufacturer's specifications. The RIDG-U-RAK Selective Storage Racking System in the New Warehouse Area was not fastened to the floor using anchor bolts.

The Powered Industrial Truck operator was not re-trained following the incident by the employer.
