29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (a) On or about June 8, 2023, the Die Caster #2 machine did not have the point of operation fully guarded to prevent access, exposing employees to crushing and amputation hazards. (b) On or about June 8, 2023, the Die Caster #6 machine did not have the point of operation fully guarded to prevent access, exposing employees to crushing and amputation hazards. (c) On or about June 8, 2023, the Die Caster #9 machine did not have the point of operation fully guarded to prevent access, exposing employees to crushing and amputation hazards.

29 CFR 1910.179(l)(3)(iii)(d):Pendant control stations shall be kept clean and function labels kept legible. (a) On or about June 8, 2023, a Budgit, 2 ton, overhead hoist located over NP 7-10 Die Cast, had a remote operated pendant control that was not legible for employees to identify the functions of the buttons, thereby exposing employees to a struck-by hazard.
