Location: 8 Ober Street, Salem, MA;  On or about 6/13/2023, employees were allowed to work directly beneath overhead roofing work and were not protected from potential falling objects by hard hats.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  Location: 8 Ober Street, Salem, MA;  On or about 6/13/2023,  the employer has not provided fall protection training for the company's employees. The employer representative on-site stated there has been no safety and health training provided to any employee in the company and was evidenced by the lack of understanding or application of fall protection systems on the jobsite.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  Location: 8 Ober Street, Salem, MA;  On or about 6/13/2023, employees were not protected from falls by the use of any form of fall protection system while performing roofing work 12-feet or greater above a lower level.  Quinde General Construction LLC dba Quinde's Construction Inc was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1432236, citation number 1, item number 3 and was affirmed as a final order on 10/24/2019, with respect to a workplace located at 335 Eliot Street, Milton, MA 02186.

29 CFR 1926.1053(b)(10): The top of a non-self-supporting ladder was not placed with the two rails supported equally; nor was it equipped with a single support attachment.  Location: 8 Ober Street, Salem, MA;  On or about 6/13/2023, the portable extension ladder at the back of the residence leading up to the main roof was not equally supported against the structure by both rails.

Location: 8 Ober Street, Salem, MA;  On or about 6/13/2023, employees were allowed to utilize the portable extension ladder at the front of the residence which had one or more broken rungs.
