29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Location: Jobsite- 321 Sycamore St, Holbrook, MA  On September 15, 2023 an employee was exposed to overhead struck-by hazards from tools and materials while working at ground level as other employees were working on the roof where protective helmets were not utilized.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Location: Jobsite 321 Sycamore St, Holbrook,  MA  On September 15, 2023, employees were exposed to falls from heights up to 25 feet to the ground while installing residential roofing materials where fall protection training was not provided to recognize and minimize fall hazards.

Quinde General Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1432236, citation number 1, item number 3 and was affirmed as a final order on 10/24/2019, with respect to a workplace located at .

OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls  Location: Jobsite- 321 Sycamore St, Holbrook, MA  On September 15, 2023, employees were exposed to fall hazards of up to 25 feet while using a ladder platform shingle hoist to access the roof. The hoist was not designed to be used in place of a ladder.  Quinde General Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard OSH ACT of 1970 Section (5)(a)(1)- using a shingle platform hoist as a ladder, which was contained in OSHA inspection number 1432236, citation number 1, item number 1 and was affirmed as a final order on 10/24/2019, with respect to a workplace located at 225 Eliot St, Milton, MA.
