On the ground, at a temporary residential job site located at 3012 Ernest Street, Jacksonville, FL 32205: On or about June 13, 2023, the employer exposed an employee to head injuries in that a hard hat was not utilized by employee picking up debris while underneath the other employees working on the roof.

On the roof of a temporary residential worksite, located at 3012 Ernest Street, Jacksonville, FL 32205: On or about June 13, 2023, the employer exposed employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by employees, while using a Craftsman blower to blow off debris from the roof.

On a 3:12 pitched roof of a residential worksite,  located at 3012 Ernest Street, Jacksonville, FL 32205:  On or about June 13, 2023, the employer exposed employees to fall hazards, in that, fall protection was not utilized by the employees while performing roofing activities.

On a temporary residential job site located at 3012 Ernest Street, Jacksonville, FL 32205: On or about June 13, 2023, the employer exposed employees to fall hazards ranging from 6 to 8-foot, in that the ladder side rails of a 14-foot portable extension ladder were not extended 3 feet above the roof eve.

At at temporary work site located at 3012 Ernest Street, Jacksonville, Florida 32205: On or about June 13, 2023, the employer exposed employees to fall hazards, in that employees were allowed to an  aluminum 14-foot extension ladder that had a bent side rail and no feet on either side.  The ladder was not removed from service or repaired.

At a temporary residential work site, located at 3012 Ernest Street, : On or about June 13, 2023, the employer exposed employees to fall hazards, in that the employees used the top step of a 6-foot A-frame step ladder to gain access and egress from the roof and was being exposed to fall hazards up to 8 feet.
