29 CFR  1910.178(a)(4):Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  (a)  On or about June 14, 2022, and at times prior thereto, the employer exposed employees to struck-by and crushed-by hazards in that employees were allowed to operate a Doosan 25 forklift with a homemade 5th Wheel / Receiver Hitch Attachment. The forklift attachment was not certified by the manufacturer of the forklift.  White Sands RV, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.178(a)(4), which was contained in OSHA inspection number 1575775, citation number 2, item number 1 and was affirmed as a final order on 03-31-2022, with respect to a workplace located at 19408 AL-59, Summerdale, AL, 36580.
